someone logged into the online account of a credit card i cancelled about 6 mos ago. they changed the billing address and have been feverishly trying to use it all weekend.

in order to log in, they had to know the my username + pw
to use the card they had to know the cc # + expiration
after logging in, they also got home address, phone number, secret question + answer, email address and my FICO score
